They started painting rank on helmet-liners mid-way through WW-II although it wasn't really common until the '50s. It was 'required' for officers starting in 1942, I have both the 1941 and 1942 publications of "The Officer's Guide" the later book has the reg and guidance for helmet-rank, the earlier one doesn't. While most are hand painted, there were decals available for purchase at the PX.

I don't know about that, but I know they requested eight digit coordinates whenever possible. That's pretty tight on a VN 1:50,000 tac map.
