get a pair of 6 lug 4x4 front rotors, and make some caliper brackets. Hope you have a lathe to make the hubs and rotors fit each other. There is not bolt on junkyard setup that I know of.
Yes, and no. Hubs can be re-drilled by your friendly neighborhood machinist. There are drill jigs for re-patterning, but those are customarily the same bolt circle. HAMB'er Rottenleonard makes jigs. He did make a 5 to 4 re-patterning jig, so it is technically possible. Used to be you could buy a kit that had a bearing spacer and a caliper bracket, and use 70s Camaro rotors and calipers. I don't know if ECI still makes the kit, that was a long time ago that I did one. You can also see about adapting later drum brakes, the 51-59 truck Bendix style drum brakes are better than the older Huck style brakes. But if you want 5 lug, you might see if you can use the 51-54 car brakes. If we had more info about the car, and what you plan to do with it, we might be able to make more useful suggestions. Disc brakes are not necessary, in many cases. I still have drums on two of my four old Chevys.
I am sure it has been done many times, but it might also be worthwhile to consider using new parts instead of junkyard parts on something as critical as brakes.
