This early sbc intake has me stumped finding it's application. Only 4 runners, 471 and 671 don't fit. Breather tube says it's early.

There was a company named LATHAM that made a supercharger with a bunch of vanes inside it and it took 4 side draft carbs. The carbs were like the early 6 cylinder Corvette carbs. I'm not sure that is what you have but maybe.

Looks like the B&M 162 supercharger bolt pattern on it, could it be an early engineering prototype?
And compare it to this early SBC fuel injection manifold, was the basic GM design plagiarized and modified for the blower?

I'll have to check out that name. I have 3 old engine builders and none of them recognize it.

  5. It's not a Latham. I've got lots of info on them.
A Latham intake for an SBC was very much like an SBC FI intake.

Here are the dimensions for B&M/Holley blowers. I’m betting on the 162 gasket dimensions matching your manifold.
