I called Red's headers and the person I talked to me said they didn't have lakes style headers for Model A's and when I asked it they had that style kits available, they said no and didn't mention the flanges. I have a Sacramento Vintage Ford catalog and didn't see anything in there about Lakes style headers.
Just build a set, Get a u-weld it header kit for a V-8 from Summit or Speedway motors and fab up some flanges, I`ve built several sets of flanges for oddball setups like that. Best part of all you will have plenty of tubing left over for the next project! And you can build them your way!

I have bought flanges from red's headers, but I don't know of anyone with off the shelf lakes headers for "A"'s or "B"'s. Antique Ford Specialties in Rosemead Ca also had some flanges as did AC&R. For my Denver Miller I just bought some 3/8's plate and made my own on my Bridgeport because of core shift and then I sent them to Red's and he built some 1 3/4 tubes. Getting this stuff done is not impossible just takes a little searching.
My flange came from Reds with some bent bits and megaphone for about $120. It is bunch of parts that have to be welded together. We'll see how it turns out. I'll probably do the fitting and tacking, but may have it finished by a pro.
