Did you run the engine at 1500-2000 rpm for 20-30 minutes to break in the cam?
If so, Ill bet all the break in lube ran off of the cam lobe while the engine was sitting...Ive never seen a lifter wear down that much before it was detected.

Just looked at the picture again...by the angle of the wear, Id say that the lifter was tight in is bore and couldnt rotate.

The lifter might have been too tight.Every lifter that I ever saw with excessive ware was worn in the shape of a hemisphere.Yours is worn on an angle which means that it was not rotating. Worst yet you have a lot of material worn off of the lifter that circulated throughout the engine. Better check all of the moving parts for signs of wear and damage. Not a pretty sight.

I had a similar issue and thought it could have been cause by a lack of zinc in today's oil. I always add ZDDP or GM's EOS now because of that.

More than that, you need to tear the engine COMPLETELY down, knock out the cam bearings, pull the galley plugs and hot tank the block to get all the worn lifter material out.
In other words, you need to start from scratch.

I agree with Pete1, when you've had that kind of a failure, it's time to take it completely down and make sure there is no more powdered lifter and cam dust anywhere in the engine.
Your oil filter can't catch all of it, and what gets by is like valve lapping compound.

AS much as I hate to make work for you I have to agree, tear it down ,clean everything. It doesn't do anygood now but if I have something fresh that has set over 6 months I tear it to the shortblock to check for cyl. rust and to relube everything.
