Technical 354 hemi broken lifter, and cam spacer

I just got a 354 hemi, very little back ground info, was set for a blower, block O ringed, Venolia 8:1 blower pistons, Clay Smith Solid flat tappet cam, on dis***emble I found a broken lifter, must not have ran long because the break was clean not hammered. when I pulled the cam retainer the spacer was broken in multi pieces, although it appeared to be in correctly, chamfered edge facing the cam. This is my first Hemi any thoughts on what might have happened.
