Technical Help identify plate

I bought this adapter for a flathead to a 39 transmission. The seller told me he had the plate also. i had my nephew pick it up for me, when i got it i noticed it is not correct. Any idea what it goes to?

Just a guess, flathead Ford six cylinder?
