Technical Stadco Life Saver Stoplight installation & wiring instructions.

Hi,
Recently I bought one of these Stadco accessories, a miniature stoplight that hangs in the rear window, that indicates if you are slowing down or stopping. The light works, but I did not get the installation instructions. I see old threads on here from long ago where some folks were willing to share a copy of theirs, but I have not got a response on those links. I see where you can buy a set off eBay, but I am hoping to get a scanned copy from someone on here if possible. So, would someone help me out?

So there is a control unit or a sliding switch that was sold with the light? Or was that an extra cost item?
